This Personal Data Processing Agreement has been developed in accordance with the requirements of the Law of Ukraine “On Personal Data Protection” and defines the procedure for processing and protecting users’ personal information.
Individuals who provide their personal data on the website or otherwise provide information about themselves confirm their consent to the processing and use of such data and, where provided for by law, its transfer to third parties.
For the purposes of this Agreement, personal data means information that makes it possible to identify an individual, including their surname, first name, patronymic, email address, and contact telephone number.
By accepting the terms of this Agreement, an individual provides voluntary, specific, and informed consent to the processing of their personal data for the purposes of providing medical services, ensuring proper communication, providing information about services, and complying with applicable legislation.
The user undertakes to provide accurate, complete, and up-to-date information that does not violate the legislation of Ukraine or the rights of third parties, and confirms that all data provided relates personally to them.
The processing of personal data includes any actions or set of actions related to such data, including its collection, registration, accumulation, systematization, storage, clarification (updating or modification), use, dissemination, anonymization, blocking, or destruction in accordance with the specified purpose.
Personal data may be transferred to third parties exclusively in the cases and in accordance with the procedure provided for by the legislation of Ukraine, provided that an appropriate level of data protection is ensured.
An individual has the right to withdraw their consent to the processing of personal data at any time by contacting the website administrator or the designated responsible person. In the event that consent is withdrawn, data processing may be limited to actions necessary to fulfill legally established obligations.
The user confirms that they are aware of their rights as a data subject, including the right to access their personal data and to have it corrected, updated, or deleted in cases provided for by law.
